• Why did I receive another employee’s Premera information?

According to Premera, there was a processing error when data was sorted for address validations. Premera has described this as a “human error” which resulted in the addresses being shifted on the enrollment census and ID cards issued to the incorrect addresses. 

• Do we know which employees are affected?

Classified Staff and Administrators who chose Premera during the most recent open enrollment period may have been affected.

• Do we know how many people are affected?

According to Premera, there are 116 subscribers and 206 members that are impacted. 

• Is this just Ferndale School District or are there other groups involved?

We have been told by our Insurance Broker (Partners Group) that Ferndale School District is the only District that has been affected at this point.

• What information was shared?

According to Premera, name, group number and insurance plan. No social security numbers and no birth dates were shared.

• What information would someone need to use my benefits?

Social security and birth date are both needed in order to activate the card and access the benefits.

• When did the Ferndale School District become aware of the situation?

Members of the Ferndale School District Executive Team received two reports late in the evening of October 15th from employees providing notification of Premera information delivered to incorrect addresses.

John Fairbairn, Executive Director of Human Resources at the Ferndale School District, called an immediate Human Resource Team meeting first thing in the morning on October 16th.

The Ferndale School District Human Resource Team has been working with Premera since we became aware of this issue and will continue reporting out to our employees as information becomes available.

• What is the Ferndale School District doing to fix this issue?

The Ferndale School District Human Resource and Payroll Teams have been working with Premera on this issue since we became aware there was a problem.

We commit to continuing to work towards the best remedy for all impacted employees and group members.

• When will this issue be resolved?

Premera has informed us that corrections have been made and cards will be re-issued to those impacted. If you do not receive your card by October 30th 2018, Please contact Shelley Carpenter at 360-383-9227 or shelley.carpenter@ferndalesd.org.

• Will Premera be re-sending cards to the correct addresses?

• What should I do if I receive a Premera Insurance packet addressed to someone other than me or a family member covered by my insurance?
